Abstract

This book, edited by a team of leading European bioethicists, is in all respects an innovative publication. As part of the core materials project of the European Ethics Network, this book collects European perspectives on health care ethics reflecting both the rich philosophical tradition and the broad interdisciplinary network in the field of European health care ethics. In the first part of the book on the physician-patient relationship, the authors present different views on the integration of patient autonomy in the relational structure of the medical profession. Here, the focus is on the reception of patient autonomy in the European context and on European alternatives for the radical understanding of patient autonomy. In the second part of this book, the contributions deal with the ethical problems of clinical research with patients and with the ethical values involved in human experimentation. In the third part of the book, the just organisation of the health care system and the consequent resource allocation are critically examined. With their common European background, the authors want to enrich and encourage the ongoing discussion on the physician-patient relationship, human experimentation and justice in health care from various perspectives and different points of departure. Therefore, this volume is suitable for the interested reader as well as for educational purposes

Depuis, il est devenu possible d'accéder aux mécanismes moléculaires de l'hérédité, de modifier les formes naturelles du vivant, et de créer des "OGM", des organismes génétiquement modifiés, bactéries, plantes ou animaux - sans que les processus biologiques mis en jeu soient toujours parfaitement compris, ni parfaitement maîtrisés. Les utilisations de la génétique réveillent dès lors des craintes anciennes, et en activent de nouvelles, qui touchent les unes et les autres aux conséquences de la manipulation du vivant, en général et de la modification de l'humain, en particulier. Ce livre analyse la manière dont notre société a accueilli ces techniques, et restitue les termes du débat qui en est résulté, pour aboutir à un constat implacable : jamais ce débat n'a été à la hauteur des enjeux. Drawing on twenty-five years of medical practice and research, Dr. H. Gilbert Welch and his colleagues, Dr. Lisa M. Schwartz and Dr. Steven Woloshin, have studied the effects of screenings and presumed preventative measures for disease and "pre-disease." Welch argues that while many Americans believe that more diagnosis is always better, the medical, social, and economic ramifications of unnecessary diagnoses are in fact seriously detrimental. Unnecessary surgeries, medication side effects, debilitating anxiety, and the overwhelming price tag on health care are only a few of the potential harms of overdiagnosis.Through the stories of his patients and colleagues, and drawing from popular media, Dr. Welch illustrates how overdiagnosis occurs and the pitfalls of routine tests in healthy individuals. We are introduced to patients such as Michael, who had a slight pain in his back. Despite soon feeling fine, a questionable abnormal chest X-ray led to a sophisticated scan that detected a tiny clot in his lung. Because it could not be explained, his doctors suggested that it could be a sign of cancer. Michael did not have cancer, but he now sees a psychiatrist to deal with his anxiety about cancer.

L'éthique et notamment la bioéthique ont-elles encore un sens aujourd'hui? Alors que l'hémisphère Nord accumule recommandations et réflexions éthiques, le Sud peine à garantir les soins les plus élémentaires à ses populations. La greffe d'un visage qui fait la une de tous nos quotidiens a-t-elle plus d'importance que la rougeole qui tue chaque année des milliers d'enfants africains? Dans ce monde d'égoïsme sauvage, où l'éthique est souvent réduite à une simple étiquette marketing, la médecine, piégée par sa technique instrumentale, prétend rendre aux malades leur dignité, oubliant que le consentement éclairé qu'elle prône n'est souvent qu'une façon d'éluder ses propres responsabilités. Et que dire du principe de précaution brandi par les praticiens comme l'art de la prudence alors que, dans le discours médical, les mots dignité ou solidarité vident chaque jour davantage de leur substance?
